About Dorsal Hump Reduction
The nasal bridge plays a significant role in the overall appearance of our face. For some individuals, a dorsal hump on the nasal bridge can cause dissatisfaction and self-consciousness about their facial aesthetics. Whether it is due to genetics or trauma, a dorsal hump can disrupt the harmony of facial features and impact one’s confidence.
Dorsal Hump Anatomy
The dorsal hump is located where the nasal bones and the bony vault connect to the cartilaginous vault (dorsal septum and upper lateral cartilages). (1) It can be addressed by hump resection, either through external or internal means. (2) During rhinoplasty procedures, hump reduction is frequently performed when the dorsal hump creates a crooked or protruding profile. It is not usually medically necessary, but performing a reduction can give patients with this issue an entirely new outlook on their overall facial structure by improving facial symmetry.
There are two commonly performed techniques of surgical rhinoplasty: external and endonasal.
Open (External) Rhinoplasty
Sometimes to properly address certain degrees of dorsal hump formation, external rhinoplasty is performed. The open approach to the surgery is usually chosen for patients that have large humps that require tissue resection, as this method uses an incision that extends around and underneath the nostrils to give the surgeon optimal visibility.
Our surgeons dissect the hump carefully. The amount taken is determined by the patient’s nose shape, the surrounding features, and the proportions of an “ideal” nose. This often involves reconstruction of the nasal tip and grafting to create an even septum structure. Each patient has a specific treatment plan to correct dorsal humps, and Dr. Greenberg will plan out his procedure accordingly. (3)
Closed (Endonasal) Rhinoplasty
Dorsal humps can sometimes be augmented by closed rhinoplasty procedures if there is minimal restructuring needed for other nasal structures. It is preferable for isolated dorsal humps. The incisions used during the method are inside the nostrils, easily concealed by the nasal structure after the patient has healed.
Though this method leaves less scarring, it can be difficult to acquire proper visibility of the bridge of the nose. Excess dorsal cartilage or bone fragments are dissected in both techniques, but they can be more thoroughly shaped during an open approach. Endonasal rhinoplasty also allows for quicker recovery times for patients. (4)

Preparation
To prepare for your rhinoplasty, the surgeons at Greenberg Cosmetic Surgery may recommend you stop consuming certain medications and herbal supplements. You should not take NSAIDs (e.g. aspirin and ibuprofen) for at least 2 weeks before your procedure, you should also refrain from using tobacco and nicotine products.
Avoid alcohol for 48 hours before your procedure. It is recommended that you pick up any prescriptions that your surgeon writes for you before the surgery date. You may also prepare an area at home that will be comfortable for recovery, and you will be required to find a trusted adult to drive you to and from our facilities for your rhinoplasty.
Dorsal Hump Reduction – Osteotomy Procedure
After the dorsal hump is reduced during rhinoplasty, an osteotomy must be performed to close what is called an “open roof deformity.” This procedure recreates the dorsal dome (bridge) by smoothing the remaining bony bridge, or at times requires additional cartilage to be added to the area.
Once an appropriate amount of dorsal cartilage or bone has been removed, your surgeon then redrapes the soft tissue back over the bridge to determine and remove any irregularities. They smooth the surface and reconstruct it with cartilage taken from somewhere else on the patient’s body if necessary. This step is important to reconstitute the entire nasal structure and add strength to your nasal bridge after dissection. He may also use spreaders to ensure your nasal canals are clear and functional after the reductions and then make any other adjustments to the nasal tip. The nose is sutured and properly wrapped. Rhinoplasty surgery usually takes 2.5 to 3 hours to complete. (1)
Recovery & Results
Reducing the dorsal hump can result in longer recovery periods for patients, especially if the bridge has to be reconstructed. After you are escorted home, expect to take at least one week off of work. Your surgeon will place gauze in your nostrils, and you may experience bleeding for 3 to 4 days after your procedure. Your nose will be placed in a splint immediately after surgery that is kept on for the following week.
Swelling and bruising will persist for the following 6 to 9 months. Though there will be fluid buildup inside your nose, avoid blowing it for as long as possible. The procedure weakens the bridge and it is important to be delicate with your nose while in recovery.
Your new nose structure won’t be apparent until your 3rd or 4th postoperative month, and your final results may not be visible until up to a year later. It takes a while for this area to heal, but once it does, you will have permanent, beautiful results for the rest of your life.
Corresponding Procedure – “Liquid” Nose Job
Non-Surgical Rhinoplasty
If your dorsal hump is small or less noticeable, soft tissue fillers can be used for non-surgical augmentation of the nose. When patients present with minor tissue defects can receive temporary but often effective filler injections to correct slight humps. Also called a liquid nose job, this is a viable option for patients who do not have severe dorsum projection. Though there is no downtime associated with this type of rhinoplasty, it is not permanent like surgical procedures.

FAQ
Can a dorsal hump come back after surgical reduction is performed?
No. A dorsal hump does not grow back after surgery, although there is a slight chance that the nose can develop calluses where the bony tissue has been removed. This side effect can sometimes resemble a “new” dorsal hump. Excessive swelling can also make it look as though your hump is growing back, but this will dissipate once you are fully recovered.
Can dorsal hump reduction correct a crooked bridge?
Yes. Rhinoplasty with dorsal hump reduction can work very well to straighten out the bridge of the nose. It is, however, important to remember that there may still be irregularities after your surgery. Aesthetic results are just as important as functional results.
What results can I expect from dorsal hump reduction surgery?
