What to Know About Scarring After a Breast Reduction Procedure

Breast reduction scars are a normal part of healing after surgery. If you are considering breast reduction, or reduction mammoplasty, it is natural to wonder where the scars will be, how visible they may look, and whether they will fade over time.

The good news is that breast reduction scars are typically placed in inconspicuous areas. Patients may experience mild scarring after breast reduction, but scars are usually concealed under clothing and tend to fade closer to the natural skin color over time.

For many breast reduction patients, the tradeoff is well worth it. Undergoing breast reduction surgery can help improve discomfort from heavy breasts by removing excess breast tissue while also creating a smaller, perkier, more lifted breast shape.

Where Are Breast Reduction Scars Located?

Your breast reduction scar tissue pattern depends on your anatomy, breast size, degree of sagging, and the amount of tissue being removed. Incisions may be made around the nipple, from the nipple to the chest wall, or beneath the breast along the inframammary crease.

Common incision patterns may include:

  • Periareolar incisions: Around the areola
  • Vertical or “lollipop” incisions: Around the areola with another vertical incision down toward the breast crease
  • Inverted T or “anchor” incisions: Around the areola, vertically down the breast, and another horizontal incision along the breast crease

These incision options are selected based on the amount of sagging and the degree of correction needed.

How Breast Reduction Scars Heal Over Time

Breast reduction scar healing takes time. In the first few weeks, your incisions are still delicate, and you may also have swelling, soreness, and bruising. Sutures are typically removed about two weeks after surgery, and patients should avoid vigorous activity and heavy lifting during early recovery.

During the first several months, scars may look pink, red, darker, or slightly raised depending on your skin tone and healing response. They may also feel tender or itchy after surgery.

By months one through six, swelling and soreness continue to improve. Proper healing and final results can take up to one year as the breasts settle into their new shape. After one year, scars begin to fade into the natural skin color.

How to Support Better Scar Healing

Your scar care plan should come directly from your surgeon. In general, the most important step is to follow your post-operative instructions closely and avoid placing unnecessary tension, friction, or irritation on your incisions, as this can disrupt your body’s healing process and make scarring worse.

Silicone gel or another scar cream may help support an optimal healing process and encourage healthy new skin. Scars can take about 12 months to fade.

You should also keep all follow-up appointments. These visits allow your surgical team to monitor your healing and tell you when it is safe to begin scar treatments, exercise, supportive bras, or other recovery steps.

Why Choosing the Right Breast Reduction Surgeon Matters

Visible scarring is part of any surgical procedure, but careful planning can make a meaningful difference. At Greenberg Cosmetic Surgery and Dermatology, our team of board-certified plastic surgeons reviews your anatomy, symptoms, breast reduction options, and surgical approaches during your consultation.

The goal is not only to reduce breast size. It is to create breasts that feel lighter, look more balanced, and heal as smoothly as possible. The right surgeon will explain where your incisions may be placed, what your scar pattern may look like, and what you can realistically expect as your body heals.

Scarring After Breast Reduction FAQs

Are breast reduction scars permanent?

Breast reduction scars are long-lasting, but they usually fade over time. Scars typically begin to fade into the natural skin color after one year.

Can breast reduction scars be hidden?

In many cases, yes. Breast reduction scars are typically hidden by a bra or swimsuit.

When can I start scar cream after breast reduction?

You should only start scar cream, silicone gel, or scar treatment when your surgeon says it is safe. Starting too early may irritate healing incisions.

What affects how my scars heal?

Your skin type, genetics, incision pattern, surgical plan, aftercare, and healing response can all affect scar appearance. Scar size and pattern depend on the technique your surgeon uses and your specific breast anatomy.

Is breast reduction worth it if I am worried about scars?

For many patients, yes. Breast reduction can reduce physical discomfort and improve breast shape, proportion, and confidence. It’s a procedure that can help patients feel as good as they look, with greater comfort and self-confidence.
